What you need to know

Every therapist requires consent forms for every client they see. I have tried to streamline this process as much as possible. If you are looking for help, you don't need to get bogged down in 30 forms just to finally sit down and fill out more paperwork. I require 4 consent forms to be signed: Notice of Privacy Practices (confidentiality and it's limits), Informed Consent (Knowing your rights), Authorization for Billing (insurance and payments), and an Emergency Contact Agreement (in case of emergencies). You will also fill out a basic demographics form that tells me a little bit about you, including any medical issues I need to know about or medication you take. You can always skip anything you are uncomfortable disclosing. You will also answer a quick survey of your symptoms so I know where to start. You must complete all forms BEFORE you can schedule an appointment.
Once your paperwork is done, you can request an appointment at the above link. Your initial evaluation is a 60 minute appointment. During that first appointment, I will ask several background questions to get a good idea of who you are as a person. Don't worry if you feel like you're "word vomiting." (That means you're doing it right.) All sessions afterwards are 50 minutes. If you arrive more than 15 minutes late for any appointment, and are using insurance, I cannot bill the session and therefore cannot see you. This means it will also count as a missed appointment.

Do you take insurance? - Yes, I use Headway as my third party insurance billing system. The insurance plans I accept are listed under my bio.

Do you see clients in person or on telehealth? - Both. Whatever works best for you.

What age range do you see? - I see adults. I am not currently accepting new adolescent clients.

How much is your self-pay rate? - $150 per session

What is your cancellation policy? - In order to avoid the cancellation fee, you must give me 24 hours notice, reschedule in the same week, or provide a doctor's note to waive the fee.

Can I pay cash? - No, I only accept debit, credit, or health savings cards.

What if I miss a payment? - If you do not pay your balance within 30 days, your remaining appointments will be cancelled until payment is submitted.

Why do you need an emergency contact? - ECs will only be contacted if I am concerned for your safety after you miss an appointment and I cannot reach you. No additional information will be disclosed to them, other than the missed appointment. I will not call for a welfare check unless in extreme circumstances.

How do I request my records? - You can request your records through email or letter. I have 14 business days to respond to any requests.
